The following forms may be printed on your printer. Please fill out the applicable form and deliver it to the State Capitol Police administration office in Room 1200 of the Legislative Office Building, 300 Capitol Ave., Hartford, CT 06106.

Flag Request: Use this form when making a request to fly a flag on the center flagpole.
ID Badge Request Form: Use this form when making a request for an ID access badge.
Room Access / Removal Request Form: Use this form when making a request to add / delete a room access to your ID badge.
Lock Change Request Form: Use this form when making a request for a key or lock change to a desk or file cabinet.
Officer Recognition Form: Use this form to recognize an officer for outstanding service.
