AN ACT REQUIRING THE USE OF THE STATE BOARD OF EDUCATION'S HEALTH ASSESSMENT RECORD FORM.
To require the form supplied by the State Board of Education be the only acceptable form for health assessment records for the purpose of school enrollment.
