In line 30, after "chemicals" insert the following:

"; and (9) for each such chemical identified (A) a list of each company doing business in the state that is known to use such chemical, (B) a description of each consumer product known to contain such chemical, (C) the name and address of each manufacturer of such consumer product, (D) the number of persons employed by such manufacturer, (E) the amount of taxes such manufacturer pays, annually, to the state and to the municipality in which it is located, (F) an evaluation of the potential impact on such manufacturer and, collectively, on manufacturers in the state that may result from a ban or limitation on the use of such chemical"
