In line 184, strike the period and insert "; and" in lieu thereof

After line 184, insert the following:

"(4) The physician has filed with the Department of Public Health, on a form and in the manner prescribed by the department, the physician's name and address, the nature of the qualifying patient's debilitating medical condition and such other information as the department may prescribe. "
