After line 98, insert the following:

"(e) Whenever a physician issues a written certification for the medical use of marijuana under subdivision (2) of subsection (a) of this section, the physician shall report such issuance to the Department of Public Health not later than three days after the date of such issuance, in such form as the department may require. The Department of Public Health shall maintain a record of all such written certifications issued by physicians under said subdivision. Not later than January 15, 2006, and annually thereafter, the Department of Public Health shall submit a report to the joint standing committees of the General Assembly having cognizance of matters relating to the judiciary and public health, in accordance with the provisions of section 11-4a of the general statutes, setting forth the number of written certifications for the medical use of marijuana issued by physicians under said subdivision in the preceding calendar year. "
